Dear Colleague,

Camden and Islington Public Health is inviting you, as a health professional working within Camden and/or Islington, to share your views on how we can support and encourage inactive patients to become and remain physically active.

Your views will inform future interventions and services for patients (given the recent end of the Exercise on Referral service in both boroughs) and help to develop support for clinicians in discussing physical activity with patients.

We would be extremely grateful if you could take ten minutes to fill in the survey.

The survey will close on Friday 2 July.

We plan to follow this survey with more in-depth conversations with health professionals around future support for both patients and clinicians. If you would be willing to participate in these discussions, please provide your email address in the relevant section at the end of the survey, or email mustafa.farooqi@islington.gov.uk to express interest.

Kind regards,

Dr Mustafa Farooqi
GP Registrar
