Age UK Islington supports adults of all ages experiencing a health or wellbeing challenge. Our Wellbeing in 1 service is a “one-stop shop” designed to help people overcome non-clinical issues e.g. debt, housing issues, social isolation. Or to help people to maintain and improve their wellbeing by keeping active and engaged.
Case work is based on the level of support required. This can range from a piece of information to intense one-to-one support over a number of weeks, including home visits, close liaison with professionals to facilitate client engagement, and practical advice and support. We can also help clients to co-ordinate care and support provided by multiple agencies. Referrals come to one point and we gauge the level of support required.
From transport, to money, to keeping safe at home, to support with a caring role - it focuses on what matters to the individual.
Casework may involve more urgent practical support such as getting repairs done or getting bills paid, alongside longer-term goals such as getting out and meeting people or getting into volunteering or employment. Whatever support Age UK's clients receive, it's about building confidence and resilience – getting ready for ‘next time’.
Age UK is well positioned to bridge the gap between health and statutory services to the myriad of things that can make a client's life better - from debt advice to dance classes. Support can include exploring options, organising transport, helping people settle in. We provide Social Prescribing Link Workers based in 14 GP surgeries in PCN South & Central 1, who deliver this support to GPs. They are also the gateway to Age UK Islington’s wider Wellbeing in 1 service, including more intensive casework support. GPs in these areas can simply refer via EMIS.
Age UK keeps in touch with clients - following up with systematic preventative support over the long term; making regular proactive phone contact with clients and providing early intervention – over months, years, decades.
