Barnet Community Anticoagulation Service

This Anticoagulation Service (LCS) will assist in Primary Care and community based services at scale to the registered population of Barnet for the management of patients aged 18 years or over, for the initiation of anticoagulation and monitoring, where agreed clinically appropriate. 

The service will cover:

  •  All suitable patients registered with a GP in Barnet. 
  • This is an anticoagulation service which includes initiation of the most appropriate anticoagulant as well as ongoing monitoring for any patients receiving warfarin or DOACs. Complex patients will remain in secondary care. 

The initial clinics, with a near patient testing service, will run from:

  •  The Speedwell Practice within Torrington Health Centre, 16 Torrington Park, London N12 9SS
  • Oaklodge Medical Centre (adjacent to Edgware Community Hospital), 234 Burnt Oak Broadway, Edgware HA8 0AP
  • PHGP Doctors, Temple Fortune Health Centre, 23 Temple Fortune Lane, London, NW11 7TE 

Eligibility criteria


Monitoring service

  • Age over 18
  • Stable anticoagulated patients – target TTR>65%
  • Minimum 12 weeks post initiation  

Initiating service

  • NVAF 


The following patients would be expected to be kept under secondary care unless there are mitigating reasons for them to be under the care of the community clinic: 

  • A known hereditary or acquired bleeding disorder.
  • Known antiphospholipid syndrome
  • Liver failure/decompensated liver disease;
  • Alcoholism
  • Documented evidence of CNS haemorrhage;
  • Gastro-intestinal bleeding in the previous 6 months;
  • IV drug user
  • Cancer patients undergoing cancer therapies (e.g chemotherapy) or with metastatic malignancy
  • Uncontrolled severe hypertension (>180/110)
  • Under 18
  • Pregnant patients
  • Patients with Mechanical Heart valves (Valvular AF)
  • Dialysis patients who have bloods done during dialysis
