The Islington Community Neurological Rehabilitation Team provides multidisciplinary, specialist neuro-rehabilitation for the adult residents of Islington. Independence is promoted, while maximising safety and managing risk. The aim is to minimise impairment, while enhancing activity, participation, wellbeing and quality of life, by being responsive and accessible and providing high-quality, integrated, client-centred services. 

The team agrees therapy goals with people and works towards these over a period of time. 

Eligibility criteria


  • residents of Islington aged 18+
  • with therapy needs as a result of a neurological diagnosis or complex physical disability including:
    • people with a diagnosis of stroke, parkinson’s disease, multiple sclerosis, brain injury, motor neurone disease, cerebral palsy, chronic fatigue syndrome, brain tumours and other neurological problems. 
    • early supported discharge from local hospitals after acute stroke.


  • referrals for equipment or wheelchair provision only

Please consider the following alternative services before referring to CNRT or REACH:

  1. Musculoskeletal physiotherapy: please refer via central bookings ( if client is able to leave their house for a physiotherapy appointment (including with transport)
  2. Falls medical clinic: please refer via GP.
  3. ICAT: For clients aged 75+ who require a comprehensive geriatric MDT assessment, please refer via emai
  4. Occupational Therapy Social Services: for equipment provision and major adaptations, please refer via Access and Advice
