Nutrition and Dietetics Community Services
Haringey’s community provider for nutrition and dietetics is Whittington Health. For patients registered with a Haringey GP and who meet the referral criteria below. All patients referred must express motivation to change. We also offer a limited paediatric service, specified below.
GENERAL NUTRITION SERVICES – Who to refer…
Diabetes
Pre-diabetes: Patients with pre-diabetes who have not responded to first line advice.
Type 1 Diabetes: Please refer to acute diabetes team.
Type 2 Diabetes: Newly diagnosed Type 2 and Poorly controlled Type 2
Include recent Hba1c, BP and Lipid results
Diagnosed Digestive Issues
Irritable Bowel Syndrome
Coeliac disease
Diverticular Disease
Functional gut symptoms (bloating/wind/constipation/diarrhoea)

Enteral Feeding Service (Adults)
The Home Enteral Feeding service supports and manages patients unable to eat orally who are tube fed via Nasogastric, Gastrostomy or Jejunostomy feeding routes. Mobile patients can be seen at any of our clinics.
Cardiovascular
Raised lipids
Hypertension
Other (please state): Include recent Hba1c, BP and Lipid results
Malnutrition /Nutrition Support
Unintentional weight loss of 5-10% within 3-6 months
Pressure ulcer
Nutritional supplement review
Ongoing poor appetite (>10days)
Dysphagia (must also be seen by SALT)
Provide current weight and weight history over the previous 3 months with BMI and/or height.
Diagnosed Nutritional Deficiency
Iron deficiency anemia
Vitamin D
Vitamin B12

Weight management
BMI > 30kg/m2 (or >27kg/m2if South Asian Ethnicity)

HOME VISITS
For adults who are housebound and are not able to be brought to a clinic via transport/car, a domiciliary visit and /or telephone consultation can be arranged with the patient, if they meet the following criteria:
Type 2 Diabetes (a one off visit)
Patients with persistent raised fasting glucose > 7.0 mmol/l or HbAIc7.5% or 58 mmol/mol, who have not responded to initial first line advice
Patients with Type 2 diabetes with a cholesterol above 4mmol/l or LDL > 2mmol/l
Patients who have poorly controlled diabetes who also have complications such as renal impairment, leg ulcers, CVD and/or hypoglycaemia.
Nutritional support
Provide current weight and weight history over the previous 3 months with BMI and/or height. If bedbound, please provide MUAC measurement where possible.
Our referral criteria for Oral Nutrition Support is as below:
MUST 2 or more (see MUST Online calculator to calculate)
Pressure sore/ulcer
5-10% unintentional weight loss within 6 months and BMI <18.5kg/m2 (link to NHS BMI Calculator) https://www.nhs.uk/live-well/healthy-weight/bmi-calculator/
5% unintentional weight loss within 3-6 months and BMI < 20kg/m2
Poor appetite lasting longer than 5 days with an acute illness
On oral nutritional supplements
Patients with Swallowing difficulties with any of the above(a-e) e.g. post CVA, dysphagia MUST be referred to a Speech and Language Therapist before referral to a Dietitian

All referrals should be made using the Nutrition and Dietetics Referral Form located in the referrals section.
