Islington's End of Life and Palliative Care Service (ELiPSe), provided by a specialist palliative care team, is for patients whose usual place of residence and place of care is within the community, including their own home.
Referrals can be made at any stage of the patient’s illness, however it is advisable that the patient’s care plan is reviewed and that the patient’s preferred place of care is observed, where clinically appropriate. Care records are available through Coordinate My Care (CMC), which is also accessible via an in-context link in EMIS.
Patients aged 18+ who are registered with an Islington GP and/or living in Islington:
- with a diagnosis of potentially life-limiting illness with unresolved complex needs that cannot be met by the referring team. These needs may be physical, psychological, social and/or spiritual
- with advanced progressive life-limiting illnesses who are likely to be in the last year of life
And their family and carers.
Patients living in Islington who are not registered with a borough GP practice should be supported to register with a local GP practice.
Out-of-borough patients cannot be treated by North Central London (NCL) service providers. Patients living outside NCL boroughs should be referred to emergency services when appropriate or signposted to their local service provider.
